He has developed healthcare partnerships and referral networks across Virginia, North Carolina, and Maryland, working with hospitals, physicians, rehabilitation providers, home health agencies, and senior living organizations to expand access to care.
At Rich Health Solutions, Bryan leads business development, referral partnerships, and community outreach initiatives that support individuals receiving Medicaid Waiver services. His focus is on building relationships that help connect individuals and families with quality therapeutic consultation and therapy services across Virginia.
Bryan's experience in healthcare growth, partnership development, and strategic outreach helps support Rich Health Solutions' mission of providing high-quality, person-centered services that promote independence, participation, and quality of life.

Kimberly serves as the Clinical Support Specialist for Rich Health Solutions, providing administrative and operational support to help ensure the delivery of high-quality services for individuals and families across Virginia.
Kimberly brings a diverse background in healthcare and leadership. She earned her certification as a Certified Occupational Therapy Assistant (COTA) through the Occupational Therapy Assistant program at Tidewater Community College and spent four years working as a COTA at Virginia Beach General Hospital. She also worked for two years in a skilled nursing facility as a temp CNA/rehabilitation aide and served for three years as a supervisor at a Target distribution center. Her experience across healthcare and operations has strengthened her organizational skills, attention to detail, and ability to support both clinical teams and the individuals they serve.
In her role, Kimberly assists with client onboarding, documentation review, compliance tracking, support plan coordination, and quality assurance processes. She works closely with the clinical team to help maintain accurate records, monitor important deadlines, and support efficient day-to-day operations. Her attention to detail, strong organizational skills, and commitment to excellence help ensure that clients, caregivers, and clinicians receive the support they need.

Caroline is a Board Certified Behavior Analyst (BCBA) with over a decade of experience supporting individuals with autism and developmental disabilities. Since 2015, she has provided direct care and behavioral support across clinical, home, and community settings, earning her BCBA certification in 2018. Her experience includes facilitating social skills groups, delivering parent training, and developing individualized behavior support strategies.
In 2025, Caroline expanded her practice to provide consultative services for caregivers of adolescents and adults with developmental disabilities. Her professional interests include caregiver training, reducing severe challenging behaviors, and supporting communication through Augmentative and Alternative Communication (AAC).
Caroline earned a Bachelor’s degree in English from George Mason University (2014) and a Master’s degree in Early Childhood Special Education from Old Dominion University (2016). Outside of her professional passion, she enjoys reading, gardening, and spending time with her three dogs.

Justin Scroggins is a Board Certified Behavior Analyst and Licensed Behavior Analyst dedicated to helping individuals with developmental disabilities build meaningful skills, increase independence, and improve their quality of life. A lifelong Virginian, Justin earned his Bachelor's degree in Human Services and Counseling from Old Dominion University and his Master's degree in Special Education with a concentration in Applied Behavior Analysis from Arizona State University.
Justin's passion for the field began at an early age after witnessing his mother's dedication to supporting family members with developmental disabilities. Throughout his career, he has worked with individuals with developmental disabilities, at-risk populations, and those experiencing homelessness. His clinical approach emphasizes assent-based, trauma-informed care and collaboration with the individual, family, and support team. Justin has also completed advanced Skills-Based Treatment (SBT) training developed by Dr. Gregory Hanley and utilizes evidence-based strategies to create individualized supports that reflect each person's strengths, needs, and goals.
His professional interests include client advocacy, skill acquisition, behavior support, caregiver training, and professional mentorship. Outside of work, Justin enjoys spending time outdoors with family and friends, practicing yoga, and spending time with his dog.

Jill Chumley is a licensed Physical Therapist with more than 20 years of experience serving individuals across a variety of healthcare settings, including acute care, home health, skilled nursing, and community-based services. Throughout her career, she has helped individuals improve mobility, increase independence, and achieve meaningful functional goals. She has also served in leadership roles as a Lead Physical Therapist and Director of Rehabilitation, developing a strong appreciation for interdisciplinary collaboration and person-centered care.
Since 2021, Jill has specialized in providing Physical Therapy Consultation services within Virginia’s DD Waiver system. She works closely with individuals, families, caregivers, support coordinators, and interdisciplinary team members to promote safety, accessibility, mobility, and participation in everyday activities. Jill believes the best outcomes are achieved through teamwork, individualized support, and a focus on each person’s unique strengths and goals.
Jill is passionate about empowering individuals and their support teams through education, training, and collaborative problem-solving. Dedicated to lifelong learning, she is currently pursuing a 200-hour yoga teacher certification to further expand her knowledge of movement, wellness, and holistic health.
Outside of work, Jill enjoys playing and singing with her band, The Relative Miners, listening to live music, playing pickleball, hiking, practicing yoga, and spending time outdoors.

Physical Therapist Consultant
Lara Perkins is a Doctor of Physical Therapy who is enthusiastic about helping people live fuller, safer, and more independent lives. With experience spanning hospitals, rehabilitation settings, home health, and community-based services, she brings a practical, compassionate, and creative approach to supporting individuals with diverse physical and developmental needs.
As a therapy consultant, Lara enjoys working collaboratively with individuals, families, caregivers, and interdisciplinary team members to overcome barriers, build on strengths, and create meaningful experiences in everyday life. Rather than prescribing one-size-fits-all solutions, she enjoys creative problem-solving and developing strategies that fit each person's unique needs, goals, and environment.
She specializes in mobility, adaptive equipment, fall prevention, and functional movement strategies that can be incorporated into real-world settings.
When she is not working, Lara enjoys spending time outdoors with her family, tackling home projects, gardening, and keeping up with the adventures of raising three young children.

Melanie is a licensed Physical Therapist dedicated to helping individuals achieve greater independence, safety, and quality of life. She brings a diverse clinical background with experience in assisted living and memory care communities, as well as acute care settings, including intensive care, orthopedics, trauma, cardiac care, and the treatment of acute neurological conditions.
Passionate about patient-centered care, Melanie works closely with clients and their families to develop individualized strategies that promote functional mobility, maximize independence, and support meaningful participation in daily activities. She is committed to empowering those she serves through education, collaboration, and compassionate care.
Outside of work, Melanie enjoys spending time with her family, relaxing at the beach, and playing with her dog.

Heather Sheets is a Certified Therapeutic Recreation Specialist with 16 years of experience helping individuals improve their quality of life through meaningful recreation, leisure, and community engagement. Throughout her career, she has worked with children, adolescents, adults, and older adults in a variety of settings, including inpatient behavioral health programs, where she facilitated recreational therapy groups designed to promote coping skills, emotional wellness, social connection, and functional independence.
Heather works collaboratively with individuals, families, caregivers, and interdisciplinary teams to support community participation, leisure skill development, independence, and overall well-being. She has a special interest in helping individuals discover meaningful activities that enhance their quality of life while building the skills needed to successfully participate in home and community environments. Her areas of focus include community integration, adaptive recreation, leisure education, social skills development, sensory-friendly programming, and supporting individuals with autism and other developmental disabilities.
Heather is passionate about creating person-centered interventions that promote confidence, inclusion, wellness, and long-term success through meaningful recreational experiences.

Kayleigh Turner is a Certified Therapeutic Recreation Specialist and a recent graduate of Virginia Wesleyan University. While obtaining her degree in Recreational Therapy, she completed internships at The Waterford at Virginia Beach Assisted Living and
Memory Care and the Spinal Cord Injury Unit of the Veteran’s Administration Hospital in Hampton, VA. In addition to working with seniors and adults in medical settings, Kayleigh has volunteered with young adults in community programs and has been working with children and adolescents for the past nine years as a gymnastics coach.
Kayleigh feels that every individual possesses unique strengths, abilities, and interests that can be used as powerful tools for growth, healing, and improved quality of life. Kayleigh takes a strengths-based approach to recreational therapy by focusing on what a person can do rather than their limitations. By identifying and building upon these strengths, she is able to empower individuals to gain confidence, develop independence, and find meaning through recreation and leisure experiences.

Carly is a Speech-Language Pathologist and Orofacial Myologist who is passionate about helping children, adults, and their families feel confident, supported, and successful. She believes therapy should be engaging, individualized, and centered around each person’s unique strengths, goals, and needs. Carly is dedicated to building meaningful relationships with the individuals and families she serves while providing evidence-based care that promotes lasting progress and improved quality of life.
Her professional interests include Orofacial Myofunctional Therapy, tethered oral tissues such as tongue, lip, and buccal ties, pediatric and infant feeding, speech sound disorders, and airway-focused care.
Carly enjoys collaborating with families and interdisciplinary providers to support functional communication, feeding, oral development, and overall health across the lifespan. Her approach emphasizes education, empowerment, and practical strategies that help individuals succeed in their everyday lives.

Brogan is a Speech-Language Pathologist who has been practicing for 8 years. She earned her Master's degree in Communication Sciences and Disorders from Old Dominion University. Her professional passions include working with individuals with traumatic brain injuries, stroke survivors, and people with developmental disabilities.
Brogan's certifications include completion of training in Modified Barium Swallow Studies (MBSS) and Fiberoptic Endoscopic Evaluation of Swallowing (FEES).
Outside of work, Brogan enjoys gardening, cooking, and taking bike rides with her family.

Sara Williams, OTD, OTR/L is an Occupational Therapist who earned her Doctorate in Occupational Therapy from the University of St. Augustine for Health Sciences, where she specialized in education. She has experience working across a variety of healthcare settings, including skilled nursing facilities, acute care, and inpatient rehabilitation.
Sara is passionate about helping individuals with physical disabilities maximize their independence and participation in meaningful daily activities. She particularly enjoys collaborating with families and caregivers, providing education and practical strategies that support improved functional outcomes and enhance quality of life.
Outside of work, Sara enjoys spending time outdoors, staying active, and making memories with her family.

Amy is an occupational therapist with over 13 years of experience helping individuals achieve greater independence, participation, and overall well-being. Throughout her career, she has worked with children and adults with a wide range of developmental, neurological, and physical disabilities in outpatient, inpatient, home, and community-based settings.
Amy is passionate about finding individualized solutions that empower clients to achieve their goals and participate more fully in the activities that are meaningful to them. She values collaborative, client-centered care and enjoys working closely with clients, caregivers, and interdisciplinary teams to identify practical strategies that support long-term success. Her background includes extensive experience supporting individuals with complex needs, as well as expertise in adaptive equipment, wheelchair seating and mobility, durable medical equipment, and home accessibility.
In her free time, Amy enjoys being active outdoors with her family and dogs, as well as traveling and exploring new places.

Sarah is an Occupational Therapist who earned her Master’s degree in Occupational Therapy from Virginia Commonwealth University. She began her professional journey with an undergraduate degree in Social Work from James Madison University in 2006, followed by experience working in a clubhouse-model day program supporting adult survivors of brain injury. This work, along with subsequent experience supporting clients in therapy-related roles, inspired her to pursue a career in occupational therapy.
Sarah has been practicing as an Occupational Therapist since 2017. She completed her clinical fieldwork in a skilled nursing facility and a pediatric outpatient clinic. Although she initially anticipated working primarily with adults, she discovered a passion for working with children and families and has since gained experience in pediatric outpatient, community-based, and consultative services.
Sarah values a consultative approach to practice, using clinical reasoning to develop individualized, functional recommendations tailored to each person's unique strengths, needs, and daily routines. She enjoys collaborating with caregivers, support teams, and interdisciplinary providers to support meaningful participation, independence, and quality of life.
Her clinical interests include fine motor development, activities of daily living, sensory regulation, caregiver education, and supporting individuals in achieving greater independence and participation in everyday activities.
